C'mon, Rock is all about hard work and devotion to his ninja dream. Is there any doubt he's going to choose to undergo surgery? It'd be a total letdown if he chose not to do it. How dare you even ask that question!! Rock Lee is not some chump girly-man that lays in a bed all day when hes injured (...sasuke...), no, Lee would rather be outside doing pushups. There is no way Lee wouldnt take the operation, hes just waiting a bit, trying to get a little bit more play from sakura (possibly to silence the growing rumors of a...special relationship between him and Gai...or just get her off sasuke's nuts a litte).

Rock Lee will definatly choose to have the surgery, hes been working so hard all his life for this goal and i bet the only way he wants to live is as a ninja, so he will say yes.
Pretty sure Rock Lee will choose to do it, also episode 99 had Tsunade studying the body so I think that's setting up for it, plus rock lee dosen't seem to be the type to give up so he'll probably choose to do it
