I believe she was specifically referring to the kinds of wounds he inflicted on himself during the Shoto fight(i.e. double injuring himself to the point where he ended up permanently scarred). I think she's still willing to heal his normal self inflicted injuries.

Also, was it established that Recovery Girl cannot heal serious injuries? Because otherwise it´s bullshit if she doesnt just heal Lida´s brother.

Well, she wasn't able to heal All Mights injury, so obviously she has limitations.
