Does anyone have any idea/theories on how to become a jounin? Is there a Jounin exam or something? Maybe if you do like an A rank mission or two, they'll promote you to jounin.

Mastered sharingan at 7, became a chuunin at age 9, Became an ANBU at age 13 (which is basically a record), Left the village at 13, in other words, wiped out the clan at 13.
no one knows about the jounin exam. i still don't think itachi ever achieved the jounin rank since he left way too early.
No, it's all about how you consider an ANBU as a chuunin or a Jounin, officially he was a chuunin when he became an ANBU, in skill and strenge he's an exellent jounin, but as you said he never achieved the title jounin, I think.
